Our Wraparound Program is a fidelity model of Intensive Care Coordination for qualifying youth under 18. The coordinator builds a client-driven, familycentered team to collaborate with natural supports, service/system providers, and family partners. The coordinator guides team to prioritize the needs of the youth and family, develop a Plan of Care, and ensure coordination with the team.
