Wraparound Program Care Coordinator

Access Behavioral Health Services (our Nampa location) is looking for dedicated professionals with Targeted Care Coordination experience. We offer extensive initial training, collaborative supervision, and a culturally competent work environment. Staff will build teams for youth who are diagnosed with serious emotional disturbance, serious persistent mental illness (SED, SPMI). Staff will form collaborative teams in a client-driven, family-centered model. Staff will work with children, their family, their supports, and their providers. Staff will follow Wraparound Program requirements. Clients will be located in the DHW Region 3. This is considered a full-time hourly position at 30+ billable hours a week.

Staff Provider Requirements Minimum Qualifications:

18 years or older

Must be able to pass a background check

Must have a car and a valid driver’s license

BA/BS Bachelor degree from a nationally accredited university (preferably in Primary Education, Special Education, Adult Education, Counseling, Human Services, Early Childhood Development, Family Science, Psychology, Sociology, Applied Behavioral Analysis, or Social Work

Ability to maintain privacy on Protected Health Information

Willingness to serve clients geographically throughout various towns in Idaho

Adaptable personality, meeting client needs in fast-paced conditions with a noticeable position

Be culturally competent, accepting, and supportive with families and program partners

Preferred Experience. Or Interested in Learning and Developing these Skills:

Working with families of differing socio-economic, cultural backgrounds

Working with youth and families with significant mental and behavioral health diagnosis and behaviors

Understanding skills of conflict resolution

Understanding skills of empathetic communication

Understanding skills of service documentation

Understanding skills of relational awareness

Understanding skills of motivational interviewing

Understanding skills of electronic appointment billing system: units, codes, and other indicators

Understanding skills of collaborative information sharing with team, simultaneously respecting privacy

Understanding skills of professional communication with doctors, therapists, and behavioral aides etc. Utilize electronic forms of communication in order to complete job duties, email, fax, cell phone, laptop, health records, etc.

Capacity to become CANS certified, and utilize CANS or other assessment tools in developing treatment plans and measuring client outcomes (see below “encouraged”)

Capacity to understand and practice the Idaho YES principles of Care

Capacity to complete the Wraparound Foundational 10 Module web-based Training from Idaho WInS Model of High-Fidelity Wraparound (HFW) 20 hours over 6 months

Capacity to be persistently engaged in Care Coordination enactment with multi-disciplinary teams

Cultivate ability in social/rehabilitative service delivery system; integration of services private/public sector agencies, courts, law enforcement, corrections, vocational rehabilitation, schools, etc.

Cultivate ability to present information to small or large groups in a structured learning situation, facilitate meetings online or in person formats, and develop team plans for person receiving services including overseeing or presenting to teams of people

Other Skills and Experience are encouraged

CANS experienced and certification, Be CANS certified

Transformation Collaborative Outcome Management (TCOM) Competency Center, Idaho CANS 3.0 Certified Certification available through DHW, COE CANS, nationally at TCOM training

Experienced in Targeted Care Coordination (TCC)

Experienced in Crisis management or CPI certification maintained

Experienced in implementing Care Coordination models

Experienced in Behavioral Intervention

Experienced in Mental Health Case Management or developmental Case Management

Bilingual Skills

Experienced in working with children, adolescents, and families in a mental health role or related field

Schedule

Availability during flexible business hours (9 AM-5 PM) with some availability after 5, not to exceed 40 hours a week. Some evening and weekend hours may be required due to family schedules.
