Case Management

Access Behavioral Health Services’ Case Management program provides treatment to adults and children with a psychosocial disability.  The Case Managers work with each participant and their treatment team to address various psychiatric, medical, housing, educational, vocational and any other needs identified by the individual.

Each participant will work directly with our professional staff to access resources to optimize their functioning.  Each Case Manager will assist participants through connecting them with community resources and teaching them the skills necessary to obtain and maintain these, assist with navigating the medical and mental health care system, promote utilization of evidence-based care and advocate for each individuals complex needs to be met.

Participants will work one-on-one with their Case Manager in their home and community to access and maintain supports that maximize stability and mental health wellness.  Access believes in providing strength-based treatment that will empower each participant in improving and maintaining their quality of life and independence.
